I know exactly how it happened, except how the seat latch was released. Either it wasn't latched completely or he accidentally hit it somehow and unlatched it. That's the only part I don't know. The rest is a 1 in bazillion chance. All the stars were aligned that day ... in a negative way if it works like that. I keep expecting one of the news agencies to do a reenactment, but haven't seen anything like that ... yet.

What a tragedy. :(

Well heck, I just did a quick Google search and found a depiction of what happened. I'm assuming the seat was unlatched somehow.
