I would think anybody in training would have a few practice throws with a dummy grenade. I say staged

Similar thing happened during ACT at Camp Lejeune. We were instructed to throw it stiff armed, rather than like a football.
Practiced a few throws with a dummy, then a real one. You should see the expression on some of the guys faces after they pull the pin and know they have a live grenade in their hands, they turn pale white and damn near pass out.
Anyway, one kid does everything right but let's the damn thing go on the upswing, grenade went straight up and landed about 2' in front of the bunker. We in the bleachers as well as the instructor and the kid hit the deck and got dust and dirt thrown all over us.
Poor kid got his ass chewed pretty good and had to run around the platoon all the way back to the barracks.
