I honestly don't think that's photoshopped. It could be, but someone would have had to spend quite a bit of time doing it.

Look up typical picture of a pregnant women, belly is perfectly round, stretched to the max.
A beer gut isn't that perfect no matter how you photo it.
Easy enough to transpose one to the other and do a little photo shop tweeking,
As a photographer, you should see that.
