If you take the old lady to the ER your old friend would cetainly understand since he saved your life and understands that. The girl of your dreams would think you are the perfect man, showing compassion for the old lady so you definitely have a shot with her later.

Have the friend give the old lady CPR for for the three minutes it takes to shag the hottie, then bag it with the hottie and grab the old lady and take her to the hospital leaving the friend with sloppy seconds, for which he should be grateful.

You could also tell him to come back and get you when done at the ER. If you're both gone, it's good. If you're both still there, it's good. If you're there alone, at least you tried and you get your car back.
